The 2025-2026 MSHSL Eligibility Brochure can be found on the Parent Portal for Veracross under "August Schools" with other medical information. The Eligibility Brochure must be completed before practice begins. The following sports are 7-12 sports and the Athletic Paperwork must be completed: Alpine Ski, Boys Golf, Girls Golf, Gymnastics, Boys Hockey, Girls Hockey, Nordic Ski, Softball, Girls Swim & Dive, Boys Swim & Dive.
Breck student athletes must have a current physical exam on file with the Health Center before the season begins. Please upload the document to August Schools from your Parent Portal. If you have questions about the Student Physical Exam Form, please contact Breck's Health Center at healthreporting@breckschool.org or by calling 763-381-8241.
Forms
Please fill out the Single Event Transportation Release Form when picking up your athlete from a game and give it to the coach. If someone other than a family member will be picking up your student, someone in the Athletic Office must sign it.

If a Middle School student would like to try out for a Varsity program where a MS team is available, parents must fill out the Request to Play Upper School Sports Form. (Baseball, Basketball, Football, Girls Lacrosse, Soccer, Tennis, Girls Volleyball)
